This is an amended report, delivered to the facility on 9/23/21 from LPA Jeanette Sanchez.

Licensing Program Analysts (LPAs) Jeanette Sanchez and Joanne Domingo made an unannounced visit to the facility to deliver complaint findings. LPAs conducted COVID-19 screening questions prior to entry. LPAs met with Director Natalie Swisse.

The investigation consisted of interviews with staff, observations and record review. It was alleged that at times, a school age teacher is left alone with 18 children.

Through interviews with staff and record review (March 2021 Child Monthly Attendance Detail Reports and staff timesheets), it was found that on 3/10/21, one of the two school age staff was not working due to illness. On this date, 20 school age children were present. It was confirmed that a single staff member,

who was listed as the teacher assistant, was left alone with 20 children for approximately 2 hours while waiting for replacement staff.

Based on LPA observations, interviews conducted and record review, the preponderance
of evidence standard has been met, therefore the allegation that facility did not maintain adult-child ratio is found to be SUBSTANTIATED. This violation poses a potential health, safety, or personal rights risk to children in care. The Child Care facility is issued a citation in accordance with the California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101516.5(b)(1): Teacher-Child Ratio. See page 2 for the cited deficiency.

An exit interview was conducted. The appeal rights were discussed and provided along with a copy of this report to Director Natalie Swisse on this date. A Notice of Site Visit was posted.

Licensing Program Analysts (LPAs) Jeanette Sanchez and Joanne Domingo made an unannounced visit to the facilty to deliver complaint findings. LPAs conducted COVID-19 screening questions prior to entry. LPAs met with Director Natalie Swisse.
Through interviews, staff indicated cell phones are only to be used during break, while children reported seeing staff use cell phones during working hours. The school age playground has a lower and upper playground with trees and structures that pose "blindspots" for providing adequate supervision. Children also stated that at times there are fights and that they know where to go on the playground so that they are not seen.
Although the above-mentioned allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time. The facility is reminded to continue to provide on-going training on active supervision to all staff who are responsible for supervising children.
An exit interview was conducted, and a copy of this report was reviewed with and appeal rights were provided to Director Natalie Swisse.
