On June 2, 2022 at 8:58 am, Licensing Program Analyst (LPA) Cindy Hamilton conducted an unannounced case management inspection at Children’s Paradise to follow-up on an Unusual Incident Report (UIR) submitted by the facility on April 21, 2022. LPA was greeted at the facility by Assistant Director Rosa Aragon. LPA explained the reason for the visit and was granted entry into facility. During the inspection, At 9:10 am, LPA toured the facility, took census, and met with Site Director Natalie Swisse to discuss the reported incident. LPA also interview staff and the child involved in the incident was not interviewed during inspection as child was not in attendance.

The UIR received on April 21, 2022, stated a child had fallen while running down the stairs of the loft in classroom #4, which resulted in the child breaking their right elbow. It was disclosed to LPA that the incident occurred while the teacher and instructional aide were present. In addition, the child was previously warned not to run down the stairs. LPA verified that the facility was operating with proper supervision and within ratio. LPA observed during the visit that the facility removed the loft from the classroom the weekend following the incident.

Based on the information obtained there appeared to be no violations of Title 22 Regulations therefore no citations were issued.

An exit interview was held with Director Natalie Swisse. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
