25 | On August 17, 2023 at 12:55 pm, Licensing Program Analyst (LPA) Jessica Rubio arrived unannounced to the facility to conduct a case management visit due to an unusual incident report received for an incident that occurred on 7/21/2023, reporting child (C1) was pushed down the slide and hit the side of their head on a pole causing a cut to C1's right temple. The injury later required three staples.
LPA met with Acting Director Ralani Donnarumma and conducted a tour and census of the facility. During the visit, LPA interviewed Acting Director, one staff (S1) and parent of C1 (P1). LPA was unable to interview C1 as C1 was not in attendance. Interviews revealed there were 14-20 children on the playground at the time of the incident with two teachers, (S1 and S2) providing supervision. Interviews revealed staff applied first aid, pressure, bandaged and observed C1 until P1 arrived to pick C1 up. Interviews revealed staff attempted to contact P1 as well as emergency contacts. P1 was not able to be reached immediately, however, P1 picked up C1 approximately an hour to an hour and half later. Interviews revealed P1 was calm, ate and drank normally while waiting to be picked up. LPA determined there are no violations and no deficiencies were cited at this time.
An exit interview was conducted and a copy of this report and LIC 811 Confidential Names List was provided to Acting Director Ralani Donnarumma. Appeal rights were also provided and a notice of site visit was emailed to Acting Director and must remain posted for 30 days.
