13 | Licensing Program Analyst (LPA), Natasha Persaud conducted an unannounced visit to conclude the complaint investigation regarding the above-mentioned allegations. LPA met with Health Services Director, Stacy Tinoco.
During the investigation, the facility was briefly tourede, records reviewed, and interviews conducted with staff, residents, and outside sources. It was alleged that staff did not keep the facility free from odors. It was reported there were devices on the walls that continually emit very strong, toxic chemical sprays throughout the lobby of the main administration building. On 05/07/25, LPA observed a small device affixed to the wall but there was no fragrance being emitted. The Executive Director’s (ED) interview confirmed the facility was contracted with a professional company that monitored the device. The devices were small diffusers that emit an oil-based fragrance, nothing toxic. The ED stated they discontinued the use of the diffuser on 04/30/25 due to one resident having an issue with it. The facility no longer has an active contract with the professional fragrance company. ED confirmed no other residents were affected by the scent. Continued on an LIC 9099C. |
