25 | On April 9, 2024, Licensing Program Analyst (LPA) Kelly Gerth and Licensing Program Manager (LPM) Carlos Martinez conducted a case management visit to address an issue separate from complaint investigation (Complaint Control #10-CC-20240326090747). LPA and LPM met with Nichelle Gissler, Regional Director, who was informed of the reason for the premise visit. A tour of the facility, both inside and outside, was conducted with assistance from the director.
During the course of the complaint investigation, the following was observed:
There is a trailor located on the school premises that was being used as a school age classroom that was not originally included in the license and does not have an approved fire clearance. LPA was informed that the trailor was being used due to a lack of space and was used for a couple years.
An approved classroom was being used, however the classroom is being renovated and was not ready for placement due to exposure to hazards. LPA observed that there were electrical outlets that did not have a cover and wiring was exposed. In addition, there were plumbing pipes that were coming out of the wall, which is a hazard to the children in care. LPA confirmed that the classroom was used several times due to a lack of available space for the school age children.
During review of child records, LPA reviewed childrens files and noted that 3 children had allergies listed in files, however 1 EpiPen was expired and the other was not on the school premises as required, per Title 22. |
