On 8/4/23 at 8:00 AM, Licensing Program Analysts (LPAs) Keturah Lane and Martha Avila, conducted an announced Pre-Licensing inspection for relocation and capacity increase with the applicant. Upon arrival, LPAs met with applicant Tamana Abdulrahemzai and also present was applicant’s landlord Adina Nooristani, who also helped with translation in Nooristani (tribal language of Afghanistan). The two-story home was toured and inspected to ensure an environment safe for the care and supervision of children. The home appears to be large enough to comfortably accommodate 14 children. Fire clearance was received on 7/31/23. The fire extinguisher, carbon monoxide detector, and smoke detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water in the home. Applicant states that there are no weapons in the home. CPR and First Aid expire in November 2023. Preventative health practices course was completed on 2/23/22 which includes lead poison prevention training. Mandated reporter training was waived due to language barriers. Staff immunization requirements were met. Required documents have been posted. The applicant has toys and equipment available. Applicant provided documents for helpers in the daycare. CPR/First aid expires for helpers Suliman Noori and Baqia Noori on 7/2025. Helpers are fingerprint cleared/associated and have negative TB in file.

Applicant will be using the following rooms for childcare: living room, dining room, kitchen and bathroom #1 (downstairs). Off limits areas include: pantry and entire second story which includes 4 bedrooms and 2 additional bathrooms and are inaccessible through the use of a safety gate at the bottom of the stairs and door-knob covers. The garage will also be off limits and is kept inaccessible through the use door-knob covers.

The home has a fenced backyard available for outdoor activities. LPAs observed 5 fence boards that were cracked, split or had rough edges and need to be replaced/repaired. LPAs observed a considerable slope behind the fence boards, so it is important to make sure the fence boards are secure and check them regularly for maintenance. Landlord stated at the inspection that she would replace the boards and send video to LPA Lane once completed.

Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPAs reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Applicant provided children's records from previous license to review with LPAs. Entrance Checklist was provided to the applicant. Applicant was reminded of requirements for child abuse and unusual incident reporting. Applicant was also reminded that corporal punishment, smoking, walkers, exersaucers, bouncy seats and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer.

LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPAs also informed the applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication.

On 7/10/23, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The following corrections are needed prior to the issuance of the license:

Once all corrections are made and proof is sent to licensing a license for 14 children may be granted. Applicant understands that proof of corrections must be submitted to Licensing within 30 days or the application may be denied. Applicant agreed to comply with all regulations and laws governing family child-care homes.
