On 02/25/25 at 2:00PM Licensing Program Analyst (LPA) Veronica Torres Gonzalez, conducted an announced Case management inspection for capacity increase. Upon arrival, LPA met with applicant Touran Safavi. Also present were co-licensee & 4 children. The 3-story home was toured and inspected to ensure an environment safe for the care and supervision of children. The home appears to be large enough to comfortably accommodate 14 children. Fire clearance was received on 02/20/25. The fire extinguisher, carbon monoxide detector, and smoke detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water in the home. Applicant states that there are no weapons in the home. CPR and First Aid for Touran Safavi expires on May 12, 2025. Saied Tabatabaei CPR/First Aid expires on December 2, 2025. Preventative health practices course was completed on 12/17/23 for Taouran Safavi which includes lead poison prevention training. Preventative health practices course was completed on 12/03/23 for Saeid Tabatabea which includes lead poison prevention training. Mandated Reporter Training was completed on 11/19/23 for Touran Safavi. Mandated Reporter Training completed on 12/21/23 for Saeid Tabatabae. Staff immunization requirements were met. Required documents have been posted. The applicant has toys and equipment available. There is a fireplace covered by a painting and blocked by the television in the living room area on the first floor. Also stairs are properly barricaded with mesh gates.

Licensee will be using the underground area for day care. On limit areas include, living room, bathroom, and bedroom 1 and patio, located on the underground area level 3. The garage is off limits and locked in the underground area. Off-limits areas are entire second floor. Area is inaccessible using stair gates. On the first floor, on-limits are the living room, dining room, and extra closet. LPA asked if there is a separate room or place for isolation when one child/ children were sick, and she stated the living room on the first floor will be used. The backyard is used for outdoor play and understands that visual supervision is always required. Verification of control of property is on file.

The licensee provided proof of control of property. Applicant, was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with licensee the LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant. Applicant was reminded of requirements for child abuse and unusual incident reporting. Applicant was also reminded that corporal punishment, smoking, walkers, exersaucers, bouncy seats and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. No corrections are needed. A license for 14 children may be granted upon final file review. Applicant agreed to comply with all regulations and laws governing family child-care homes.

Exit interview conducted and report was reviewed with the licensee Touran Safavi.

A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
