On August 15, 2022 at 10:31 a.m. Licensing Program Analyst (LPA) Leilani Curtis conducted a case management inspection. LPA met with Licensee Janet Andrade and toured the facility. There were 33 children present with 7 staff members. Appropriate ratio was observed. Staff members have the required background clearances and are associated to the facility.

LPA interviewed staff #1 (S1) and the director. This morning, 8/15/22, between the hours of 8:00 and 8:29 a.m. two staff members (S1 and S2) provided care and supervision to up to 26 children at one time. The facility was operating out of ratio. The director states that one staff member called out sick and another staff member arrived late.

An exit interview was conducted with Director Brandy Pearce. Director Pearce was provided with a copy of this report and her appeal rights (LIC 9058 01/16). LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit.

The director states that she will have a staff meeting to discuss ratio requirements and other title 22 regulations. The director will send LPA a copy of the meeting agenda and staff sign in sheets via email by POC due date of 9/2/22.

Based on staff interviews and record review the facility was out of ratio on 8/15/22 between the hours of 8:00 am-8:29 am when 2 staff members were supervisig u[p to 26 children at one time. This poses a potential health and safety risk to the children in care.
