On 1/28/25 at 12:15 pm, Licensing Program Analyst (LPA), Gerald Poindexter, conducted an unannounced annual inspection. LPA met with Fardowso Mohamud, office assistant/facility representative. The facility’s licensee, Anab Hade, was not present during the inspection. The school-age program typically operates Monday through Friday from 2pm-6pm, and from 1 pm on Wednesdays, when children have half -days of school. No children were present at the facility during the inspection. According to Ms. Mohamud, the facility has not accepted children since November 2024, currently has no children enrolled and no staffing, and is seeking a new director before resuming operations. Licensee, Anab Hade, arrived at the facility as the report was being reviewed with Ms. Mohamud.

LPA toured the facility, consisting of one room opposite the preschool play area. The indoor and outdoor activity space utilized for the school age children was inspected. Due to rainfall the previous day and weekend, a portion of the ceiling was collapsed with debris scattered throughout the room. LPA advised Ms. Mohamud to submit an Unusual Incident Report to the Department and report to the Department about repairs and physical changes to the room. Aside for the water damage, the room was orderly and a comfortable temperature during this visit. Adequate ventilation, heating and air are available. The furniture, books, games and toys are safe, age-appropriate and in good repair. All required forms were posted. The bathroom’s hand washing and toileting areas are in a safe, sanitary and operating condition. Facility provides pm snacks. The menu was not poster due to lack of enrollment. Food is stored in the preschool kitchen next door, separately from any chemicals or cleaning products. First Aid kit is kept in a backpack that is hang up high on the wall by the side door. Facility has an operating smoke alarm and carbon monoxide detector. Isolation area is the Director's office. Fire clearance was granted on 08/02/2021.

The school age program was approved by the Department for a waiver request on 11/12/21 granting access to the enclosed preschool play yard on a schedule basis from 2 pm. Facility has age-appropriate play

equipment that are stored in the storage area and there is a shaded structure located at the outdoor area. Children will have access to water outdoors and indoor by bringing their own water bottles from home. Facility will also have extra water bottles.

There were no current staff or children’s records for review. LPA observed no school-age children present. LPA reminded Ms. Mohamud of required staff qualifications and requirements for children's records. Facility is reminded that the Mandated Reporter Training is to be retaken every two years and can be accessed at the following website: www.mandatedreporterca.com. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

No deficiency was cited. Exit interview conducted and report was reviewed with Fardoso Mohamud and Anab Hade. A notice of site visit was given to licensee and must remain posted for 30 days.
