13 | On 3/30/22 Licensing Program Analyst Michael Morales-DeSilvestore made an unannounced initial 10-day visit, for the complaint received on 3/21/22, regarding the above allegation. LPA met with Director Sherri B. Present in the facility were 68 children with 5 staff members. During the visit, children were napping and appropriate ratios were observed.
Based on the information obtained during interviews and documentation reviewed it is determined that the allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulationse deficiency is being cited on the attached LIC 9099D. The Notice of Site Visit was provided, and LPA observed posting. Licensee is advised it must remain posted for 30 days. An exit interview was conducted, A copy of this report and Appeal Rights (1/16) were discussed and provided. Signature at the bottom of this report confirms receipt. |
