13 | On 2/15/23, at approximately 9:13am, Licensing Program Analysts (LPAs) Saraliz Velando and Selina Siao conducted an unannounced complaint visit for the purpose of delivering findings for a complaint received on 12/8/22. LPAs met with the new Director, Patricia Munoz and toured the facility. There were 6 staff and 46 children present. This incident occurred on 12/7/22. The child was being supervised while playing outdoors by a staff member that was relieving the regular teacher for a 15-minute break. The child asked the staff member to use the restroom and decided to wait for the regular teacher to return from her break. When the teacher returned from her break, the relief staff let her know the child needed to use the restroom. While the teacher rounded up the children, the child had a potty accident.
Based on the information obtained from parent and staff interviews, it is determined that staff did not assist in toileting and the child was not accorded dignity. The preponderance of the evidence has been met and therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, regulation number 101223(a)(3). The deficiency is being cited on the attached LIC9099D. Exit interview was conducted with director, Patricia Munoz. Appeal Rights and licensing report was reviewed with the Director. A notice of site visit was provided and must remain posted for 30 days.
