There is a cement walkway at the entrance of the playground. Director states there was a sensory table causing a collection of sand on a cemented area in the play area. The sand made the surface very slippery. The play structure known as a sensory table that was collecting the sand has been removed from the facility. The children in both incidents experienced scrapes and cuts. Immediate care was given. LPA observed sand and holes on the rubber flooring on the playground during inspections completed 6/17/25 and 8/26/25. LPA observed multiple holes. The holes are a tripping hazard. Director states sand was also being placed in the holes in the rubber flooring of the playground area, as an attempt to keep the holes from being a hazard. This action contributed to sand accumulating on the rubber flooring, exacerbating the hazardous conditions thereby increasing the risk for injury. LPA found the sand on the cement and rubber flooring to be slippery.
The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be Substantiated. California Code of Regulations, (Title 22, Division 12, Chapter 1) the deficiency is being cited on the attached LIC 9099D. The Notice of Site Visit was provided, and LPA observed posting. Director is advised must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Stassia House. A notice of site visit was given and must remain posted for 30 days. |
