On 9/2/25 at 11:44 a.m., LPA Renita Rodriguez made an unannounced visit, for the complaint received on 6/12/25, regarding the above allegations to continue the investigation and deliver findings. LPA met with Director Stassia House. LPA observed ratios: 11 children with 4 staff.

During this visit LPA toured the facility, conducted interviews and reviewed records. It was alleged "Staff does not properly report incidents involving daycare children", when a child gets hurt or has an incident, staff fill out an "Ouch Report" and give it to the director. Parents/caregivers are told about the incident through the application (app) the facility uses to communicate. Staff can also talk to the parents/ caregivers in person when they pick up their child. If a child needs medical care, the facility reports it to the Community Care Licensing Department by phone call within 24 hours. The facility sends a written report within 7 days.

It was alleged "Staff do not properly maintain the facility", each staff member is responsible for maintaining the cleanliness of their assigned classroom.

Cleaning tasks are performed before children arrive, throughout the day as needed, and after children leave at the end of the day. A professional cleaning crew services the facility once a month after hours. Director states the cleaning crew was providing service twice a month up until March 2025 and now only provides service once a month. The cleaning crew focuses on deep cleaning and maintaining areas not covered by daily staff routines. The areas include common areas such as the hallways, staff bathrooms, and high touch surfaces outside the classrooms. The outdoor areas are to be maintained by the facilities maintenance staff member. LPA obtained cleaning service transactions document dated for 4/30/25 and 5/30/25.

It was alleged "Staff do not provide adequate food service", Interviews conducted provide that the parents/caregivers of the infants are responsible for providing all food, meals, snacks, and drinks for their infants each day. This includes formula, if needed. Items are clearly labeled by staff if not brought in labeled. Director states snacks are provided to the older infants by the facility. Parents are asked to provide additional snacks for the center to keep at the facility.These snacks are to be brought in on the first day of the infant starting. There are snacks and lunch items available to provide to infants if it was not provided by the parent at drop off. Infants requiring formula have an emergency kit kept at the center in case extra is needed and used for the day. On 6/17/25, 8/26/25 and 9/2/25, LPA observed items in the kitchen and classrooms. LPA reviewed the parent handbook which states lunches are the parent's responsibility. The facility provides snacks only.

Although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegation(s) are found to be Unsubstantiated. Exit interview conducted and report was reviewed with the Director Stassia House. A notice of site visit was given and must remain posted for 30 days.
