NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 09/13/2022 at 9:25am, Licensing Program Analysts (LPA), Samantha Clenista and Leilani Curtis conducted an unannounced Post Licensing inspection. Upon arrival, LPA’s met with Owner, Joanne Paz. LPA's conducted a tour of the facility and observed 22 children with 3 staff. Purpose of this visit is to follow-up and inspect facility’s permanent outdoor playground that they had been working on since facility was originally licensed on 04/27/22. On 04/27/22, LPA issued a provisional license (expiring 10/01/2022) due to their fire clearance being provisional and facility was still in the process of establishing a permanent outdoor playground. LPA received a regular fire clearance on 09/09/2022. During the inspection, Ms. Paz expressed that she would like to apply for an increase capacity due to adding the outdoor playground in addition to the already licensed indoor playground. Facility is currently licensed to care for up to 24 children (ages 2-K) in the following rooms: 1, 6, 12 and 17. Facility has an approved playground waiver on file to allow the facility to use Room 12 as an indoor playground. This indoor playground was measured by LPA Clenista on 04/12/22 and it measured at 1,338.74 sq. ft, sufficient for 17 children at one time. Facility has added an outdoor playground for the preschoolers since then. LPA’s inspected and measured the preschool outdoor playground during today’s inspection. Playground is located in the parking lot, left of the building (facing it). Area was observed fully fenced, which was higher than 4ft. tall. The infant and preschool play area is separated by a 2 ft. fence. Ms. Paz is reminded to keep the fencing dividing the infant and preschool playground where LPA's observed it today. LPA’s observed the preschool area to have umbrellas and canopies used for shade and had age appropriate play equipment. Area was observed to be free from hazards and in good repair. Preschool side of the playground measured at approximately 1,442.05 sq. ft., sufficient for only 19 children at one time. Ms. Paz stated she has plans to purchase and install turf and play structure(s). LPA asked Ms. Paz to submit pictures after the installation to LPA Clenista.

Due to the facility requesting an increase capacity from having 24 to 45 children, LPA's inspected and observed a total of two bathrooms that include 4 toilets (4 separate stalls) total, and 3 sinks. One of the bathrooms are located in Room 11 (which is an infant room). There are two separate stalls with one toilet in each. One stall has a portable sink and is designated for staff use only. The other stall and sink in the bathroom is designated for children use only. Both stalls and the children sink are labeled. Ms. Paz stated that children will be escorted to and from bathrooms. LPA's reminded Ms. Paz that direct visual observation must be maintained at all times.

Facility is requesting that the indoor playground be counted in addition to the outdoor playground. Areas of both combined equals 2,780.79 sq. ft., sufficient for 37 children at one time. Due to the facility requesting a license for 45 children, the following is needed:

Change of Capacity application (LIC200A)
Updated facility sketch for both indoor and outdoor activity space
$25 processing fee (check or money order)
Waiver request to share the outdoor and indoor playground on a scheduled basis
Playground schedule showing that no more than 19 children will utilize the outdoor playground at one time and no more than 17 children will utilize the indoor playground at one time

Before the department can issue a regular license, a playground waiver must be submitted and approved. A license for 45 preschool children in Rooms 1, 6, 12 and 17, maybe be granted upon receiving the above corrections, playground waiver approval and final file review. Exit interview was conducted with Ms. Paz and Ms. Hewitt. Notice of Site Visit was provided and is to remain posted for 30 days.
