Facility had parent meetings and made efforts to partner with parents, provided additional teacher training, developed behavioral logs to track and understand behaviors, additional supervision during outdoor activities were added, children’s problem solving and appropriate touching curriculum was incorporated, and ultimately, parents would be asked to pick up their child if incident warranted, and notified continued aggressive/impulsive behaviors would jeopardize child’s enrollment.
Investigation interviews conducted and classroom observations provided insufficient evidence children bathroom use are unsupervised. During site visits on 4/12/24, 4/23/24, and 5/23/24 staff was observed supervising one or multiple children using the bathroom and helping when requested.
Investigation interviews indicate coat racks bolted against the wall have fallen on a couple of occasions in a couple of classrooms and were quickly remounted by maintenance but has never been a potential hazard to children when used as intended. During site visits on 4/12/24, 4/23/24, and 5/23/24 coat racks were secure and observed in good repair.
Based on the information obtained during interviews and documentation obtained there was insufficient evidence that parents were not notified of incidents observed involving their children. Some families did not respond to LPA’s efforts to contact them, therefore some incidents could not be confirmed or denied if they occurred or if they were notified. Per regulations, parents may be notified of incidents verbally and/or provided written notices by the end of the day.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations occurred, therefore the above allegations are found to be Unsubstantiated.
Exit interview conducted and report was reviewed with the Director Cheryl Molenkamp. A notice of site visit was given and must remain posted for 30 days. |
