13 | On 3/16/2021 @ 3:00 p.m. LPA Joelle Redding conducted a virtual visit due to Covid 19 restrictions. The purpose of this visit is to deliver findings on the above-referenced allegations. During the investigation LPA conducted interviews with Director, staff and potential witnesses and reviewed pertinent documentation. Per the Director, Child #1 was left behind, sleeping in her classroom, during an evacuation due to a fire on the hill behind the facility. She was retreived approximately 5 to 7 minutes later when it was determined that she was not with her group. Further, the Director stated that she was unaware that the evacuation and the child left behind was to be reported to Licensing. Based on the information obtained via interviews and review of documentation, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Deficiencies per California Code of Regulations, Title 22, are being cited on the attached LIC 9099Ds. Appeal Rights (1/16) were discussed. Per Covid-19 regulation, the Licensing report and Appeal Rights are being delivered via email. Facilty's reply to this email is considered confirmation of receipt. |
