On 3/16/2021 @ 3:00 p.m. LPA Joelle Redding conducted a virtual visit due to Covid 19 restrictions. The purpose of this visit is to deliver findings on the above-referenced allegations. During the investigation LPA conducted interviews with Director, staff and potential witnesses and reviewed pertinent documentation. Per the Director, Child #1 was left behind, sleeping in her classroom, during an evacuation due to a fire on the hill behind the facility. She was retreived approximately 5 to 7 minutes later when it was determined that she was not with her group. Further, the Director stated that she was unaware that the evacuation and the child left behind was to be reported to Licensing. Based on the information obtained via interviews and review of documentation, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Deficiencies per California Code of Regulations, Title 22, are being cited on the attached LIC 9099Ds. Appeal Rights (1/16) were discussed. Per Covid-19 regulation, the Licensing report and Appeal Rights are being delivered via email. Facilty's reply to this email is considered confirmation of receipt.

Director states that following the incident, she addressed staff with regard to new protocol regarding evacuation procedures and an emergency drill was conducted on 1/21. Each classroom is now equipped with an emergency backpack containing snacks, water, first aid kit and emergency contact information. Director will incorporate transition and supervision

Director's confirmation that Child #1 was left behind, asleep in the classroom, for approximately 5 to 7 minutes during an evacuation prompted by a fire on the hillside behind the school. This is an immediate hazard to children in care.

Director has already submitted the Unusual Incident Report as of 1/29. Director states that she will submit a signed, written statement that she has read and understands the requirements of Title 22 Regulation Section 101212, by the plan of correction date of 3/23/21.

Director informed LPA during an interview that she was unaware she was to report that a child was left behind during a fire evacuation on 1/14/21. This is a potential hazard to chidlren in care.

Note: Per Assembly Bill 633 (Parent Notification Requirements) the facility is to provide a copy of this Licensing Report to the parents of all children currently enrolled as well as any children newly enrolled over the next 12 month period. Parents are to sign form LIC 9224, Acknowledgment of Receipt of Licensing Reports and the form is to be kept in each child's file for Licensing's review. In addition, this Licensing report is to be posted along with the Notice of Site Visit for 30 days.
