The facility currently has a fully fenced playground area. Fencing is wrought iron with added bamboo, which is at least four feet high. The total square footage for the preschool outdoor / school-age outdoor activity space is 10209.15, which is sufficient to accommodate the requested school-age capacity. Per site director the facility currently has a waiver on file for the school-age and preschool children to share the outdoor listed preschool playground using a time schedule. The outdoor playground listed on the facility outdoor sketch is listed as preschool playground and there is no school-age playground listed. LPA's will need to verify the discussed waiver. Shade is provided via trees and building overhang. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground for children ages 2-12, which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber cushioning material. Drinking water is available via filtered in an igloo with disposable cups. LPA's observed all hazardous items on the playground to be inaccessible to children. Director Erica Gonzalez was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

1. An updated outdoor sketch will be needed to show the outdoor playground will be for preschool and school-age.

2. LPA's will verify if an outdoor playground waiver was granted for the school-age children to share with the preschool children. If a current waiver is not on file, the facility will need to request the waiver and it will need to be approved.

The requested increase from 21 to 26 school-age children ages 5-12 years of age in room #4 will be granted upon a final file review.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview was conducted and report was reviewed with Director Erica Gonzalez.
