Licensing Program Analyst (LPA), Kelly Gerth conducted an annual inspection as part of a compliance review. This is a combination childcare center, and the other licensed programs are: 376300744 and 376300743. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:
· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating within the limits as stated on the license.
· Ratios are being met during this inspection, 3 fully qualified teachers and 2 aids present with 24 children.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
· There are no weapons present at the facility as stated by Director Rosa Mendoza.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Water dispenser supplies drinking water in the indoor activity space with disposable cups and
· Drinking water is provided in the outdoor play area by water dispenser and disposable cups.
· Medications are stored where inaccessible to children, located in the Directors office in an emergency back pack.
· Hazardous items are stored where inaccessible to children which include disinfectants, cleaning solutions and other items that are dangerous

·Poisons and toxins are locked and inaccessible to children
· All floors were observed to be safe and clean.
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment. The structure is not in good condition.
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed. CCC currently using electronic parent signatures.
· Disaster drills are conducted at least every six months – last drill was conducted on 03/05/2024.
A review of staff and children's records were conducted as part of this evaluation.
· Children’s records were found to be complete during this inspection.
· Staff record review indicates that all staff present meet minimum qualifications for the position for which they were hired.
· A staff member is present with current Pediatric CPR/First Aid which expires on 06/2025
· Opening and closing staff member’s CPR/First Aid expires on 10/2025
· Director completed Health and Safety Training
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Director Rosa Mendoza was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.


For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP)

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA referred Director Rosa Mendoza to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Testing conducted on 10/05/2023.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Director Rosa Mendoza was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Director Rosa Mendoza confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

An exit interview was conducted, and this report was reviewed with the Director Rosa Mendoza.
Appeal rights were discussed and provided during the exit interview.

Based on interview and observation the licensee did not comply with the section cited above in 1/1 play structures on the shared playground has more than 10 holes and/or breaks in the plastic throughout the base of the structure that connects to the rubber mat. Additionally more than 3 areas, approx larger than 14inx14in, once stepping onto the base of the structure, are covered with wood that is in disrepair and splintering. Plastic mat that covers this wood is peeling off of the wood, which all pose a potential health, safety or personal rights risk to persons in care.

Director Rosa Mendoza will immediately use caution tape around structure and position a teacher to ensure that it is off limits and not used by children, making this inaccessible to children. Additionally, Director Rosa Mendoza states that the owners/board of the CCC will meet on 08/01/24 to discuss a plan for the outdoor playground and by COB 08/02/24, Director Mendoza will submit this plan to Community Care Licensing and detail how the playground deficiency will be made safe for children in care.
