On the above date and time Licensing Program Analyst (LPA) Kelly Gerth made an unannounced visit and met with Director Britney Eischen to deliver the findings from a complaint made to Community Care Licensing (CCL) on May 19, 2025. The complaint CCL received stated the following allegation: Staff wrongfully suspended/expelled child.
On 05/22/25 Community Care Licensing made an unannounced visit to open the complaint investigation at the facility. During this time, LPA Kelly Gerth met with Director Rosa Mendoza, toured the facility, took census, collected pertinent documentation, conducted interviews and verified enrollment and facility staff. On 06/19/2025 LPA Gerth made an additional attempted visit to confirm details and on 06/23/2025 confirmed further details with the CCC and concluded interviews.
Regarding the allegation Staff wrongfully suspended/expelled child. During the investigation, LPA Gerth found evidence of parent meetings facilitated by the CCC, documented classroom support plans, and ongoing observation notes and behavior logs for child 1 (C1). All interviews conducted corroborated what information was provided to the CCC for the student file. See Continuation Page

Therefore, based on evidence collected and corroborating interviews made by LPA Gerth during the investigation, LPA did not find a preponderance of evidence to prove the alleged violation occurred, therefore the allegation is UNSUBSTANTIATED.
A copy of this report, appeal rights and Notice of Site Visit were provided to Director Britney Eischen and was reminded that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
