25 | On date and time listed, Licensing Program Analyst (LPA) William Chancellor arrived unannounced to Children's Choice Academy (CCC) to conduct a Case Management Visit in response to an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on June 4, 2024. The incident involved a child being restrained due to behaviors by a Applied Behavior Analysis (ABA) therapist.
Documents relevant to the UIR were provided and confidential interviews were conducted with Interim Director and two staff.
LPA has determined that the facility acted appropriately, and no violations have been cited. Parents were notified at pick up, facility documented a UIR to CCL and the volunteer therapist no longer provides services at the CCC. LPA has determined this to be a random isolated incident where the child was inappropriately restrained due to behavior concerns. No other incidents have occurred involving this therapist prior to this incident. Parent of C1 immediately took action to remove this therapist from the case.
Based on the information gathered, there appears to be no violations of Title 22 Regulations found at this time, and therefore, there were no deficiencies cited during this inspection. A technical violation will be given.
An exit interview was conducted, and a copy of this report was provided to Interim Director Jenni Grawvunder. A Notice of Site Visit must be posted for 30 consecutive days.
