On 2/11/25 at 9:50 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit at the facility. On 2/10/25, LPA Lane became aware of a violation of regulation against the preschool and infant licenses. Due to time constraints, LPA was only able to cite under the infant license. On 2/10/25, LPA observed two armed guards on the premises, however facility has no approved waiver for this matter under either license. Licensee stated they did not know they needed to request a waiver from CCLD.

Upon arrival, LPA Lane was greeted by Director Naomi Shakhman and Assistant Director Romy Rothstein and toured the facility. LPA observed a total of 54 children with the following ratios:

  • Room Multipurpose room (also known as Room #121 - Yellow) had 10 children with staff members Teresa Gonzalez & Gali Tamir
  • Room 122 (Green) had 8 children with staff members Vivian Maya and Yasmin Cardona
  • Room 123 (Blue) had 9 children out on playground with staff members Lori Simmons/List and Bonita Rocha
  • Room 152 (Gold) had 9 children with staff members Judit Sponga and Liraz Abraham
  • Room 153 (White) had 8 children with staff members Sylvia Bratt and Sandra Gutierres
  • Room 156 (Purple) had 10 children out on the playground with staff members Chi "Diana" Kieu and Gratzia Kasis

ยท Room 154 was not in use (shared area for activities)

All staff were fingerprint cleared and associated to the facility. LPA inspected playground and observed the grounds were free of debris and potential hazards. (continued on LIC809-C...)

Based on LPAs observation and interviews with Director and Licensee, the licensee did not comply with the section cited above in 2 armed guards on premises without applying for a waiver, which poses a potential health, safety or personal rights risk to persons in care.

Director stated she would send the construction documents, facility sketch and e-mail stating that the Department will be notified prior to construction/alterations of the physical plant and also upon completion. Director will send items requested above via e-mail to Keturah Lane by 2/14/25.

LPA Lane became aware that there had been construction during August 2023, which involved adding a wall and door between rooms Green #122 and Yellow #121 (Multipurpose room). The construction was done while the school was closed. The Licensing Department was not notified prior to the construction, nor once completed.

Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiencies were cited (refer to LIC 809-D).

Exit interview conducted and report was reviewed with facility representative Director Naomi Shakhman. Notice of site visit was provided and must remain posted for 30 days.
