25 | On October 30, 2025, at 9:10 a.m., Licensing Program Analyst (LPA) Cindy Meier conducted an unannounced Case Management visit for the purpose of follow-up on an Unusual Incident Report that was submitted to the San Diego Regional Office. LPA met with Facility Representative, Site Director, Kristen Cobb and discussed the purpose of the inspection and was led on a tour of the facility. There were forty-three (43) children present with eight (8) staff members at the time of the inspection.
On 10/27/2025 around 10:26 a.m. Staff 1 (S1) was transitioning eleven (11) children from the TK’s classroom to the preschool playground, which requires walking through the two’s playground. When S1 opened the door to the two’s playground, S1 observed child (C1), from the two’s class, sitting and playing in the wood chips. No other children or staff were present. S1 called Director to come and assist. Staff 3 (S3) had earlier transitioned the two’s class from the playground back to the classroom. According to Director and S3, the face to name procedure had not been conducted and it was determined that C1 was left unsupervised on the playground for approximately 12 minutes. C1 was found to be calm and unharmed. Director stated an Unusual Incident Report was emailed to Licensing on 10/27/25. Director stated staff are trained in face to name procedures for transitions and are to record the number of children during transitions.
During this investigation, LPA obtained facility records, incident report, testimony, documentation, transition logs and interviewed Director. C1 was not interviewed due to age.
Based on the evidence obtained in reports and interviews, it is determined that a Lack of Supervision violation occurred as the C1 was unsupervised for 12 minutes.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, a deficiency was cited; see LIC809D.
