On 05/14/2026 Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegations. LPA met with Director Celia and informed her of the reason for the visit. There were 58 children present with 6 staff members at the time of this inspection.

During the course of the investigation, LPA conducted three unannounced inspections, and interviewed the Reporting Party, the Director, staff members, daycare children, and daycare parents. Additionally LPA reviewed facility records, including incident reports and Child Supervision Records, and reviewed photos and documentation submitted by reporting party.

During interview, Director denied the allegations. Director stated there was an incident that occurred on 01/29/2026 around 3:50 PM, on the playground, when two children engaged in a tug-of-war over a toy truck and Child 1 (C1) accidentally hit himself in mouth with the truck, injuring a front tooth. Director stated C1’s parent expressed concerns regarding supervision during this incident. Director stated the staff were in ratio at the time of the incident and she was also present on the playground during the incident and she attended to C1’s injury. Director stated C1’s injury was an unfortunate accident but was not due to lack of supervision.

Director stated on 02/23/2026, C1’s parent again expressed concerns regarding supervision on the playground due to C1 having wood chips in their diaper after recess. Director stated she was present on the playground that day as well, with two teachers, and C1 seemed happy and did not show any signs of discomfort during recess. Director stated diaper changes are usually done after recess, however on 02/23/2026, C1 was picked up while the children were still on the playground. Interviews with staff provided similar information to the information provided by the Director. Interviews with daycare children and parents did not provide any substantiative evidence to support the allegations. LPA review of Child Supervision Records for the dates/times of the incidents revealed an adequate staff to child ratio on the playground.

Based on the information obtained in interviews and record review, the allegations are found Unsubstantiated. A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

Exit interview conducted with Director and copy of this report and appeal rights were provided. Notice of Site Visit must be posted for thirty (30) days.
