25 | On 12/7/2023 at 2:20pm an office meeting was conducted with Licensee Representative Tammi Reliford to discuss recent complaint allegations and citations issued. Present at the meeting was Director Jessica Dorn, Licensing Program Manager (LPM) Cynthia Gray and Licensing Program Analyst (LPA) Adrian Castellon.
The facility has received ten (10) complaint allegations within the calendar year, including the following: personal rights, ratio, physical plant, reporting requirements, food service, and background clearance. Out of the ten allegations, two were substantiated and the facility was cited two Type B citations for being out of ratio and for a personal rights violation. LPA and LPM reminded Licensee Representative and Director that the plan of correction for the Type B citation issued on 10/20/2023 has not been submitted. Licensee Representative stated the Plan of Correction will by submitted by 12/11/2023.
In addition to the complaint violations, the facility was cited a Type B citation for care and supervision and a Type B for personal rights violation on 6/2/2023 during a case management inspection. It is noted that the director corrected the deficiency in a timely manner. Director stated that all staff members have received retraining and reviewed form LIC613A Personal Rights.
LPM discussed the above citations with Licensee Representative and Director. Licensee Representative and Director were provided the following regulations: Sections: 101214 Accountability, 101215 Administration Qualifications and Duties, 101215.1 Child Care Center Qualifications and Duties, 101229 Responsibility for Providing Care and Supervision, and 101223 Personal Rights and Health and Safety Code 1596.956 (a)(4) Ratio.
Licensee Representative and Director were informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee Representative and Director stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.
