On 11/16/2022 at 1:05PM, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. LPA met with Director Gail Filkins shortly after arrival as she was out on lunch. When Director arrived LPA proceeded to tour the facility. The following ratios were observed: Yellow Ducks (2-3 year old class) had 6 children napping on cots supervised by teacher aide Rebecca Agost. There were 11 toddlers napping in the Owls Room on cots supervised by teacher Vanessa Figueroa and toddler teacher aide Kelly Hatz. Facility is within staffing ratio. All required notices, forms and licenses were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water available for children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cots and each cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Snack menu is posted. Adequate food is available for snacks. Children brings their own lunch from home. Cleaning supplies are kept out of reach of children. Outdoor play areas is a fenced playground and there is no climbing structure at the preschool and the toddler playground. The preschool yellow ducks playground has a large tree use as shade and the toddler playground has a tarp use as shade. Equipment is age appropriate and separated by age groups, infant and preschool. The grounds are free of debris or potential hazards. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office.

Personnel and client records were reviewed, and staff members have the required immunizations and completed the online mandated reporter training. Updated needs and services forms were available for review. Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 10/20/2022. Facility has an operating carbon monoxide detector. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The facility appears to be in substantial compliance.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.

Exit interview conducted and report was reviewed with the Director.
