On January 16, 2025 at 3:35 PM, an office meeting was conducted to discuss deficiencies cited on 07/16/2024 and 08/07/2024. Present at the meeting were District Leader, Sandra Freni and Center Director, Ashley Enwia, Licensing Program Manager (LPM) Tulam Vu, Licensing Program Analysts (LPAs) Gloria Gonzalez and Jacqueline Macias.

On 07/16/2024 during a case management inspection, the facility was cited Type A, Responsibility for Providing Care and Supervision. It was determined that on 07/09/2024 two staff members left a child unattended on the playground for approximately 3 minutes.

It is noted that the Director corrected the deficiency in a timely manner. Facility Representative provided the department with a plan of correction on 7/18/2024, including a summary of the Licensing videos, stating staff were retrained for transition purposes, and training was provided to staff in a staff meeting on 7/16/24. A Training verification form was also provided with staff sign-in signatures.

On 08/07/2024 during an inspection, the facility was cited a Type A, Teacher-Child Ratio. During the meeting LPM discussed the appeal response letter dated 11/05/2024 with modified regarding the notice of deficiency issued on 08/07/2024. The decision was to uphold the violation as originally cited, however, it has been determined that the violation is categorized as a Type B violation rather than a Type A violation. During todays visit Facility Representative and Center Director were provided a Type B citation regarding Teacher-Child Ratio. Refer to LIC809D.

LPM discussed with Licensing Representative and Center Director regarding staffing procedures to ensure compliance with Teacher-Child Ratio requirement are being met at all times.

Facility Representative came to the classroom and removed one child to be taken to another classroom immediatly to be within ratio. Facility representative will provide the department a plan of correction by 8/8/24 that states the understanding of this regulation and how the facility will assure this will not be repeated.

Based on observation, the licensee did not comply with the section cited above in LPA observed Staff #1 be alone with 13 daycare children, which poses a potential health, safety or personal rights risk to persons in care.

LPM and LPA discussed with Licensee Representative and Center Director the above serious deficiency, transitioning procedures and Section 101229 Responsibility for Providing Care and Supervision to prevent future violations which will lead to civil penalty assessment.

Center Director stated that ongoing quarterly training in the above areas, for staff by utilizing CCL's resources (CCLD website training videos and Title 22 Regulations) and a plan of operation related to these areas. Center Director stated that name to face procedures have been implemented, and additional checks of the playground when transitioning to and from the playground.

Technical Support Program (TSP) services/training was discussed with Licensing Representative and the Center Director during the meeting and advised Center Director to register for Provider Information Notifications (PINs). Center Director is interested in the TSP program.

District Leader and Center Director were informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee Representative and Center Director stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.

A copy of this report was provided to Director. An exit interview was conducted and the report was reviewed with Facility Representative, Sandra Freni and Director, Ashley Enwia.
