25 | On January 16, 2025 at 3:35 PM, an office meeting was conducted to discuss deficiencies cited on 07/16/2024 and 08/07/2024. Present at the meeting were District Leader, Sandra Freni and Center Director, Ashley Enwia, Licensing Program Manager (LPM) Tulam Vu, Licensing Program Analysts (LPAs) Gloria Gonzalez and Jacqueline Macias.
On 07/16/2024 during a case management inspection, the facility was cited Type A, Responsibility for Providing Care and Supervision. It was determined that on 07/09/2024 two staff members left a child unattended on the playground for approximately 3 minutes.
It is noted that the Director corrected the deficiency in a timely manner. Facility Representative provided the department with a plan of correction on 7/18/2024, including a summary of the Licensing videos, stating staff were retrained for transition purposes, and training was provided to staff in a staff meeting on 7/16/24. A Training verification form was also provided with staff sign-in signatures.
On 08/07/2024 during an inspection, the facility was cited a Type A, Teacher-Child Ratio. During the meeting LPM discussed the appeal response letter dated 11/05/2024 with modified regarding the notice of deficiency issued on 08/07/2024. The decision was to uphold the violation as originally cited, however, it has been determined that the violation is categorized as a Type B violation rather than a Type A violation. During todays visit Facility Representative and Center Director were provided a Type B citation regarding Teacher-Child Ratio. Refer to LIC809D.
LPM discussed with Licensing Representative and Center Director regarding staffing procedures to ensure compliance with Teacher-Child Ratio requirement are being met at all times. |
