On 09/23/2025 Licensing Program Analyst (LPA) Hanna Lucas, and Hector Canton, made an unannounced visit to deliver the findings on the above allegation. LPA conducted the initial complaint inspection on 09/11/2025. There were 24 infants and 8 staff members within the infant rooms (Honeybee and Elephants) today.

During the investigation, LPA observed several sites of mouse droppings, including but not limited to: between the toy shelves, the art shelves, and underneath the cribs, in the infant rooms. Interviews with the Director and the staff members confirmed that the facility is aware of the mouse droppings and has hired a pest control company to address the issue. At the time of the initial complaint inspection, the Director provided the LPA an invoice for the most recent inspection on 09/03/2025. However, in the description there was no mention of mouse droppings or any ongoing effective treatment for rodents. During today's visit, the Director provided another invoice, dated 09/12/2025, that specficed mouse droppings were found and temporary bait stations were installed.

Despite pest control efforts, the presence of mouse droppings in areas occupied by infants were observed by the LPA, which allows for potential contact and an unsafe environment. The preponderance of evidence standard has been met; therefore, the above allegation is found to be substantiated.

A Type B deficiency is cited on the attached LIC 9099-D... This is a repeated violation within a 12-month period. A civil penalty of $250 will be assessed on the accompanying LIC 421.

An exit interview was conducted with the Director, Jessica Williams. Appeal rights and a copy of this report was provided and discussed. A Notice of Site Visit was provided and must remain for 30 days.
