On March 19, 2025, at 2:45 p.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Required- 3 year Inspection and met with Licensee, Carrie George. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Also present at the facility during this inspection was Licensee’s husband, Gregory George and four (4) day care children. One (1) of those children is an infant. This facility is a one story, three (3) bedroom, three (3) bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: Patio (day care room), bathroom and backyard. Off limits areas: Living room, den, dining room, kitchen, master bedroom with included bathroom, Bedroom #1 & #2. Off limits areas are made inaccessible by the use of child proof gates. Hours of operation are: Monday- Friday 6:00 a.m.- 6:00 p.m.

The fire extinguisher, smoke detector, and carbon monoxide detector were inspected and met requirements. Hazardous items were made inaccessible to children during the inspection. The licensee has toys, play equipment and materials available. The home has a fenced backyard for outdoor play. Licensee stated she provides direct supervision at all times when children are conducting outside activities. There is a in-ground pool located in back yard, which is fenced with a 5 feet high mesh fencing with no openings wider than four inches, and in no way obstruct a complete and clear view of the pool from the outside. In addition, pool fence has a gate that is self-closing, self-latching and swings away from the pool, per LPA observation. LPA observed a covered and locked jacuzzi in the backyard. LPA observed and inspected safety equipment to does not meet Assembly Bill 2866 standards. LPA observed a 19in life ring not labeled as US Coast Guard approved, a pool alarm that meets F2088 specifications, and safely pole with a body hook that extends beyond 12 ft.

There is a fireplace located in the living room that is securely screened. Licensee stated there are no firearms or ammunition in the home.

A review of staff records on this date indicates that all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse clearances.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee does not have a current CPR/ First Aid certification due to medical exemption however, Gregory George has an active First Aid and CPR certification expires on 3/23/2026 and is present in the home all of the time. Licensee has required immunizations on file. Licensee does not have completed Mandated Reporter Training. Facility roster is maintained and was reviewed. The last fire drill were conducted and documented on 12/12/2024.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA provided and discussed the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was also provided information regarding SIDS, Lead exposure and Shaken Baby Syndrome.

Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPA and Licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

LPA advised Licensee if there is an unusual incident to report to call Licensing within 24 hours and to follow up with an LIC624B within 7 days. Incidental Medical Services (IMS) policy was discussed with Licensee, Carrie George. Licensee states she does not currently administer any IMS at this time. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the Licensee that prior to making alterations or additions to the home or grounds, the Licensee shall notify the Department of the proposed change.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The maximum capacity for a large family child care home: 12 children (with a qualified assistant) with no more than 4 infants; or (with landlord consent) 14 children (with a qualified assistant) with no more than 3 infants, 1 child enrolled in kindergarten or elementary school and 1 child at least age 6 including children under age 10 who live in the licensee's home. Licensee provided documentation of proof of control of property for review by the Department.

A Notice of Site Visit (LIC 9213) was given to Licensee, Carrie George and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided.

One (1) type B California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D.

Two (2) technical violations issued.

An exit interview was conducted, and the report was reviewed with Licensee, Carrie George.
