25 | On 07/9/24 at 3:40 PM, Licensing Program Analyst (LPA) Sherlynn Banas conducted an unannounced annual inspection with the Licensee. Upon arrival, LPA met with licensee, Herminia Marinez and provided the Inspection Checklist (LIC126). Miguel Angel Martinez (licensee’s husband) came at 4:00 PM. The two-storey home was toured and inspected to ensure an environment safe for the care and supervision of children. Licensee stated she has 3 children enrolled and 3 were present today. The hours of operation are from 7:00 AM. To 5:00 PM. The fire extinguisher and smoke detector/carbon monoxide detector in one meet requirements and are operational. The fire extinguisher is stored by the kitchen and is accessible anytime needed. All hazardous items were latched/locked and secured out of reach of children. Licensee states that there is a weapon in the home which is in a cabinet in a safe with a combination lock. Licensee's husband stated that the safe can't be opened because they forgot the combination lock. They have not opened it for a long time and would like to sell it.
Licensee has provided adequate space for the children. Areas used for childcare include: living room, dining room, kitchen, bathroom, backyard, and backyard. Off limits areas include the garage, and the second floor. The second floor is barricaded by a safety gate. There is a working phone at the facility. The licensee has sufficient age appropriate, safe, toys and equipment available. Licensee owns the home.
Licensee’s First Aid and CPR certifications expire on August 26, 2025. Licensee meets immunization requirements. Licensee declines the flu vaccine. Mandated reporter training expires on July 13, 2025. Children records were reviewed as well as the roster. Required documents are posted. Last fire drill was conducted on June 3, 2024.
