25 | On May 25, 2023 at 2:25 p.m. Licensing Program Analyst’s (LPA’s), Leilani Curtis and Sherlynn Banas conducted an unannounced Annual Inspection and met with Licensee, Angela Abayan. LPA’s provided the LIC 126, Entrance Checklist to Licensee. Also present was the licensee’s helper Alfeia Termulo. The licensee’s husband arrived at the facility at approximately 3:30 p.m. There were 11 children in care, 2 who were infants. Facility was observed operating within ratio and capacity. LPA's conducted a tour of the home inside and outside per facility sketch. Licensee is using the following areas for daycare: family room, kitchen, dining area, living room, downstairs bathroom and enclosed rear yard. Off-limits areas include: entire upstairs and formal dining room.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Home is clean, orderly and has adequate ventilation. Children’s toys and play equipment are available and observed free of hazards. Stairs are barricaded. LPA’s observed an infant child standing in a crib which contained a pillow and blanket. There is a working telephone/email address. All cleaning compounds, detergents, medications, and poisons are made inaccessible through latches, locks, and/or placed up on high surfaces. The fireplace is screened. The fire extinguisher and smoke and carbon monoxide detector are operational. Licensee states there are NO firearms or other weapons in the home. The outdoor play area is fenced. Licensee is advised to provide direct supervision when using the rear yard and to ensure that the moveable fence is erected to ensure that the BBQ and storage area is inaccessible to children. There are no existing bodies of water present. Children records were reviewed for Emergency Information. One infant child is missing an infant sleep plan and immunization's. There are no new adults living or working in the home over the age of 18 years. All adult residents and helpers have submitted or been cleared for criminal record and child abuse index clearances or exemptions. Pediatric CPR and First-Aid certificates are valid through 10/02/2023 for Licensee. Licensee and Helper have completed the Mandated Child Abuse Reporting-per AB1207. LPA’s reviewed certification and they are in compliance. Immunization records per SB792 were reviewed and are in compliance. The last documented disaster/fire drill occurred on 2/10/2023. |
