13 | On 10/04/2024 at 10:50 a.m. Licensing Program Analyst (LPA), Adrian Castellon conducted an unannounced complaint inspection to deliver the findings of the above allegation. LPA met with Licensee, Christine Perez and advised licensee of the purpose of the inspection and conducted a tour of the facility. There were eleven children present during the inspection. During the course of the investigation, interviews were conducted with licensee, facility staff, daycare parents and daycare children. Texts, pictures and video were obtained and reviewed. The facility roster was obtained and reviewed by LPA. Due to conflicting information obtained throughout the course of the investigation, LPA was unable to determine whether or not the allegation occurred. LPA telephoned the reporting party to conduct an interview and to interview child. Reporting party did not return texts or calls. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.Exit interview conducted and report was reviewed with Licensee, Christine Perez. A copy of this report, along with Appeal Rights (LIC9058 03/22), were provided. A Notice of Site Visit was given and must remain posted for 30 days. LPA observed that the notice of site visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
