25 | On April 21, 2023, at 11:30am Licensing Program Analyst (LPA), Adrian Castellon, conducted an unannounced Case Management Inspection due to an incident with child #1. On 04/19/2023, the Department received the incident report from the facility for child #1. The facility submitted an Unusual Incident Report via email to our Agency on 04/19/2023.
During today's case management inspection, LPA met with licensee Grecia Molina and facility assistant Elizabeth Castillo. Present during today's inspection were nine children in care.
The facility self reported on 4/19/2023 that child #1 ran from the facility after stepping off the school bus that transported child to the facility. Child was greeted at the door of the bus by facility assistant Castillo and as they were walking to the door of the facility, child ran across the street. Child was on the sidewalk for about ten minutes as assistant tried to convince child to return. Licensee arrived and was able to grab child and mother of child was called. Mother arrived within 10 minutes of being called. Mother called 911 and police interviewed licensee, daycare child and mother. The Chula Vista police department came to investigate the incident on the same day and issued an Event Report number to the mother. Licensee was advised to get a copy of the report. The child did not sustain any injuries. . LPA interviewed licensee and assistant regarding the incident. Incident was reported in a timely manner.
No deficiencies issued during today's visit. LPA conducted an exit interview with the licensee. |
