13 | On July 11, 2022, at 10:20 AM, Licensing Program Analyst (LPA), Diana Sanchez made an unannounced complaint inspection to deliver the complaint investigation findings for the above allegations. LPA met with provider Raquel Macias and explained the purpose of today’s inspection. Current census is 4.
This agency has investigated the above listed allegations. Investigation was conducted by an investigator from the Department’s Investigations Branch and LPA. During the course of the investigation, licensee, facility staff, parents and children were interviewed. Facility records, medical records, child welfare services report and police reports were reviewed.
It was alleged that on 03/22/22, a teenage daycare child (C1) overdosed on narcotics while in care and therefore, the day care home does not provide a safe environment for daycare children. It was determined, that on the evening of 03/22/22, C1 was given permission to spend the night at the facility, for the first time ever. C1 transitioned from a daycare child to a friend of the licensee’s spouse’s minor child. |
