On March 20, 2025, at 8:45 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual/ Random Inspection and met with Licensee, Cheryl Moore. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Also present at the facility during this inspection was Licensee’s husband Bryant Moore and six (6) day care children. One (1) of those children was an infant. At 10:24 a.m. and additional infant arrived. This facility is a one story, five (5) bedroom, three (3) bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: living room, dining room, kitchen, hallway bathroom #1 bedroom #1& #2, den (daycare area), backyard. Off limits areas are bedroom #3 & #4, Master bedroom, bathroom #2, master bathroom, and garage. It is made inaccessible through use of door knob covers. Hours of operation are: Monday through Friday from 6:00 a.m. to 5:00 p.m.

The fire extinguisher, smoke detector, and carbon monoxide detector were inspected and met requirements. Hazardous items were made inaccessible to children during the inspection. The licensee has toys, play equipment and materials available. The home has a fenced backyard available for outdoor activities at time of inspection. Licensee has an in ground pool that is surrounded by a wrought iron fence with openings no larger than 4 in wide, with self-latching, self-closing gate that swings away from the pool and meets Title 22 requirements. LPA observed a pool alarm, a 20in life ring that is approved by the US Coast Guard, and a safety pole with a body hook that is 12 ft in length per, AB2866. Licensee has daily inspection log for LPA review. Licensee stated she provides direct supervision at all times when children are conducting outside activities. Licensee stated there are no firearms or ammunition in the home. A review of staff records on this date indicates that all assistants and other individuals who require caregiver background checks have received criminal record and child abuse clearances.

Based on interview and record review, the licensee did not comply with the section cited above in by not completing sleep log for one (1) infant in care past the age of 12 months which poses a potential health, safety or personal rights risk to persons in care.

Licensee stated that she was told that sleep logs did not have to be kept for infants older than 12 months. LPA reminded Licensee of safe sleep regulations. Licensee stated she will preform sleep log check for all infants in care until 24 months and will submit proof to the Department for the week of 3/24-3/28/25 no later than 4/3/2025.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee’s First Aid and CPR certification expires on 09/2025. Licensee has required immunizations on file. Licensee completed Mandated Reporter Training expires on 02/28/2024. Facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 11/20/2024.

During the inspection, LPA observed two( 2) cribs for infants who is unable to climb out of the play yard. Play yards are free from all loose articles and objects. Licensee physically checks on sleeping infants up to 24 months of age every 15 minutes however does not document on a sleep log. An Individual Infant Sleeping Plan [LIC 9227 (3/20)] is maintained for each infant up to 12 months of age. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA provided and discussed the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was also provided information regarding SIDS, Lead exposure and Shaken Baby Syndrome.

Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPA and Licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

LPA advised Licensee if there is an unusual incident to report to call Licensing within 24 hours and to follow up with an LIC624B within 7 days. LPA discussed and provided Licensee with the following: child care advocates email address: childcareadvocatesprogram@dss.ca.gov.

Incidental Medical Services (IMS) policy was discussed with Licensee, Cheryl Moore. Licensee states she does not currently administer any IMS at this time. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the Licensee that prior to making alterations or additions to the home or grounds, the Licensee shall notify the Department of the proposed change.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

On 12/17/2025, licensee submitted an application for a large license for up to 14 children. A fire clearance for 14 children was granted on 1/21/2025. During today’s inspection, LPA observed the facility has ample space to care for up to 14 children. Licensee stated she has an assistant that has all the required documents to work at this facility.

The maximum capacity for a large family child care home: 12 children (with a qualified assistant) with no more than 4 infants; or (with landlord consent) 14 children (with a qualified assistant) with no more than 3 infants, 1 child enrolled in kindergarten or elementary school and 1 child at least age 6 including children under age 10 who live in the licensee's home. When there are no Qualified Assistants present, the capacity reverts to the requirements for a Small Family Child Care. Applicant provided documentation of proof of control of property for review by the Department.

A Large Family license may be issued upon final file review.

A Notice of Site Visit (LIC 9213) was given to Licensee, Cheryl Moore and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided.

One (1) type B deficiency, California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D.

An exit interview was conducted, and the report was reviewed with Licensee, Cheryl Moore.
