25 | On 9/29/22 at 10:30 AM , LPAs Annette Sutherland and Patrick Ma made an unannounced Case Management Inspection due to reported Lead Exceedance levels in the facility's water. LPAs met with Director, Leslie Scheck. There were 52 children and 17 staff present in the preschool rooms.
Water outlets reported with 5.5 ppb or greater lead exceedance levels as follows:
Sink Faucet - Classroom 5 (6.90 UG/L)
Sink Faucet - Classroom 8 (11.00 UG/L)
LPAs observed "Do not use" signs being posted by the sink faucets during visit. Originally Director submitted pictures of sink faucets that were taped off and had "out of order" signs taped but has since replaced the faucets and began using them prior to retesting results. Director stated she is still waiting for the retest results and will no longer use the sinks until the results show the sink faucet is below the exceedance lead level.

LPA Annette Sutherland and LPA Patrick MA informed facility representative Leslie Scheck that this report dated 9/29/22 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA's Annette Sutherland and Patrick MA informed the facility representative Leslie Scheck to provide a copy of this licensing report dated 9/29/22 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. |
