32 | Regarding the allegation, child sustained unexplained injuries while in care, confidential interviews and record review revealed S1 did document a scratch that S1 observed on C1, however, it was not believed that the scratch occurred at CCC and there was no known incident that occurred at the CCC in which C1 would have obtained the scratch. Other information obtained revealed it could not be determined what the rash on C1’s fingers was but it may have been eczema and it was also observed and documented by S1.
Regarding the allegation C1’s feeding plan was not being followed, confidential interviews and record review revealed C1’s feeding plan was to have solid meals and three bottles per day. Interviews further revealed C1’s parents provided both the solid meals and bottles for C1 and CCC did not provide C1 any other food during the time C1 attended. Interviews revealed that S1 and S2 would offer C1 three bottles per day however, C1 would sometimes not want to drink all or any of a bottle. As a result, there were some times when C1 only drank two bottles. Records reviewed revealed that on 2/14/2022 and 2/23/2022, C1 had two bottles and also ate other baby food and solid meals throughout that time. Record review of all other days C1 attended revealed C1 drank 3 bottles.
Based on confidential interviews conducted and records reviewed during the investigation, the allegations that C1 sustained unexplained injuries while in care and C1's feeding plan was not being followed may have occurred, however is not supported or proven by evidence. Therefore, the above allegations are unsubstantiated. A copy of this report, LIC 811 Confidential Names List and appeal rights were given and explained to Owner Deseree Armstead.
