25 | On 09/17/2024 at 11:01 AM, Licensing Program Analysts (LPAs) Sumayya Habeebulla and Kelly Gerth conducted an unannounced visit to the facility. LPA Habeebulla conducted a separate case management report for this visit.
During the visit, LPA Habeebulla reviewed staff files, reviewed documents, and spoke to facility representative Deseree Armstead. It was noted during the file review that S1, S2, and S3 who were the only staff present upon LPAs arrival did not hold a valid CPR & First Aid training. The training present on file had expired and were not renewed as of this date. LPAs inquired with facility owner who stated that the facility is currently working on renewing staff CPR & First Aid training.
Additionally, it was noted during file review that S2, and S3 did not renew their Mandated Reporter training. The certificates present on file were expired.
During the tour of the infant classroom, it was observed by LPAs that the sheets used on the mattresses for the cribs were not tight fitting. LPAs observed three mattresses with loose fitting sheets, as per facility representative the sheets are provided by the parents. Facility representative was informed that the sheets are required to be tight fitted and the facility is responsible in ensuring it fits the mattress tightly. As per facility reports it was noted by LPAs that facility was given a technical violation dated 09/20/2023 by LPA Messerschmidt during the last visit for loose fitting sheets on mattresses used in the infant classroom.
For the above reasons, the facility is being cited for deficiencies, See LIC809D for cited deficiencies of the California Code of Regulations, Title 22 and Health & Safety Code.
An exit interview was conducted with facility owner Ms. Deseree Armstead. The appeal rights were discussed and provided along with a copy of this report to facility owner Ms. Deseree Armstead, on this date. A Notice of Site Visit was also provided and must be posted for 30 days.
