On the above date and time listed, Licensing Program Analysts (LPAs) Kelly Gerth and Sumayya Habeebulla and Licensing Program Manager (LPM) Pauline Beschorner conducted an unannounced visit to deliver the findings from a complaint made to licensing on 09/12/2024. Department staff met with Licensee Deseree Armstead
On 09/17/24, LPA Gerth and Habeebulla opened the investigation. At this time, LPA’s conducted confidential interviews, gathered pertinent documents and extended the investigation for further review.
Regarding the allegation that staff use equipment to restrain children in care and staff do not provide infant toys, specifically that children are placed in highchairs for long period of times and children are not provided toys to play with. During the initial inspection on 9/17/2024 at 11:09AM, LPA Gerth observed C3 sitting in highchair. C3 appeared to have just eaten as there was still some food left on their tray. At 11:54AM C3 was still sitting in a highchair. LPAs observed the children to be in a highchair or in a crib from the time of arrival to the LPAs departure at 2:10 PM. Based on interviews conducted, interviews revealed since licensing was present staff did not use any toys and placed all the children in highchairs and in cribs or on mats.

Once the children woke up the children were then placed back in the highchairs. Interviews also revealed that there is not a set schedule for the infants and the infants use the preschool schedule.
Based on the LPA’s observation, record review and corroborating interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

A copy of this report, appeal rights and Notice of Site Visit were discussed and provided to Licensee Deseree Armstead.

Licensee was reminded that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
