On 10/23/2024, Licensing Program Analysts (LPA) Kelly Gerth, and Licensing Program Manager (LPM) Carlos Martinez conducted an unannounced POC visit to the facility.

During the POC visit, LPA Gerth and LPM Martinez verified the corrections upon receiving the written statements outlined in the Plan of Correction dated 09/23/2024.

An exit interview was conducted with facility owner Ms. Deseree Armstead.

The appeal rights were discussed and provided along with a copy of this report to facility owner Ms. Deseree Armstead, on this date.

A Notice of Site Visit was also provided and must be posted for 30 days.
