25 | On 03/10/25, Licensing Program Analysts (LPAs) Sumayya Habeebulla, and Kelly Gerth, conducted an unannounced POC visit to the facility.
During the POC visit, LPAs Habeebulla and Gerth toured the facility inside and out to verify the corrections and the following was noted.
1. For the deficiency cited on 11/21/2024, Licensee agreed to submit a signed and dated statement agreeing to ensure that the bedding provided to the infant will be stain free and not soiled. Department did not receive the POC therefore LPAs arrived to verify the corrections. LPAs observed that the infants present were getting ready to nap on mats on the floor and the bedding used were stain free. Deficiency has been cleared.
2. For the deficiency cited on 11/21/2024, Licensee agrees to adhere to the limitations of the capacity as stated on the waiver and submit a signed and dated written statement of compliance.
Licensee emailed LPA Habeebulla stating that they no longer adhere to the waiver since they have a separate playground for the infant children. Upon review of the documents available at the department, there have been no reports stating the licensing of the infant playground. The infant playground will need to be measured and evaluated to ensure the compliance of the Title 22 regulations. Licensee will be required to submit an updated facility sketch and request to license the infant playground and should seize use of the playground until licensed. Deficiency has been cleared.
An exit interview was conducted with Licensee Deseree Armstead. The appeal rights were discussed and provided along with a copy of this report to Licensee Deseree Armstead, on this date. A Notice of Site Visit was also provided and must be posted for 30 days.
