On 03/10/25, Licensing Program Analysts (LPAs) Sumayya Habeebulla, and Kelly Gerth, conducted an unannounced visit to the facility. LPAs toured the facility inside and out and during the tour of the facility the following was observed –

Upon arrival at the infant care, LPAs observed Child #1 & Child #2 on the highchair while staff #1 was attending to another child and Staff # 2 was with the napping infants. LPAs checked up on the infants on different times and it was observed Child #1 was still on the highchair sitting idly. Facility will be sited for a repeat violation of allowing infants to be placed on highchairs for a prolonged period.

During the tour of the facility, staff associations were verified, and it was noted that Staff #2 was not present on the roster. LPAs inquired with Licensee stated that Staff #2 is a sub, and it was their first day. LPA contacted the department to verify the clearance and association of Staff #2 and it was noted that Staff #2 was cleared but not associated to the facility.

An exit interview was conducted with Licensee Deseree Armstead. The appeal rights were discussed and provided along with a copy of this report to Licensee Deseree Armstead, on this date. A Notice of Site Visit was also provided and must be posted for 30 days.

Licensee agrees to review the regulation and submit a written statement of a plan that will work best for the infant classroom to ensure infants are not left on highchairs for a prolonged period of time before or after meals or activities.

Based on observation and interviews, the licensee did not comply with the section cited above in allowing infants to be placed on highchairs for a prolonged period of time before and after meals or activities which poses a potential health, safety or personal rights risk to persons in care.
