13 | On 4/08/2025, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegations. LPA met with Director, Jacqueline Serrano, and informed her of the reason for the visit. There were 51 children present with 9 staff members.
During the investigation LPA conducted three unannounced inspections of the facility, interviewed Director, Assistant Director, teachers, and daycare parents, and reviewed facility records.
During interviews it was revealed that an unusual incident occurred on 01/15/2025, when Staff 1 (S1) was providing one-on-one supervision of Child 1 (C1) in the facility lobby. While S1's attention was diverted to assisting a parent who had arrived in the lobby, C1 ran out the center front doors and proceeded to run down the steps leading to the parking lot, before being stopped halfway down the steps by an exiting parent. C1 was retrieved by S1, found to be unharmed and returned to their classroom until C1's parents arrived a short time later. |

LPA met with Director, Jacqueline Serrano, and informed her of the reason for the visit. There were 51 children present with 9 staff members.
During the investigation LPA conducted three unannounced inspections of the facility, interviewed Director, Assistant Director, teachers, security staff and daycare parents, and reviewed video footage and facility records.
Based on information obtained interviews and record review, there was conflicting information received regarding the above allegation, and LPA was unable to determine if the above allegation did or did not occur, thus this allegation is deemed Unsubtantiated.
Exit interview conducted and copy of report and appeal rights were provided to the Director. A notice of site visit was given and must remain posted for 30 days.
