On 4/08/2025, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegations. LPA met with Director, Jacqueline Serrano, and informed her of the reason for the visit. There were 51 children present with 9 staff members.

During the investigation LPA conducted three unannounced inspections of the facility, interviewed Director, Assistant Director, teachers, and daycare parents, and reviewed facility records.

During interviews it was revealed that an unusual incident occurred on 01/15/2025, when Staff 1 (S1) was providing one-on-one supervision of Child 1 (C1) in the facility lobby. While S1's attention was diverted to assisting a parent who had arrived in the lobby, C1 ran out the center front doors and proceeded to run down the steps leading to the parking lot, before being stopped halfway down the steps by an exiting parent. C1 was retrieved by S1, found to be unharmed and returned to their classroom until C1's parents arrived a short time later.
