On 7/19/24 at 10:20 AM Licensing Program Analyst (LPA), Sherlynn Banas conducted an unannounced visit to follow up on a self reported incident that occurred on 6/27/24 at approximately 2:00 PM wherein child #1 fell from the structure. Upon arrival, LPA was granted entry by Daisy Aguilar (Admin. Assistant) who was at the front lobby area. Ms. Daisy called Director, Laura Clements and met with LPA Banas. Seven classrooms were toured, and appropriate ratios were observed. Room 1 has 2 teachers and 11 children; Room 2 has 2 teachers and 13 children; Room 3 has 2 teacher and 12 children; Room 4 has 2 teachers and 22 children; Room 5 has 2 teachers and 14 children; Room 6 has 2 teachers and 19 children; and Room 7 has 3 teachers and 26 children, All staff members present has the required background clearances and are associated to the facility. Census during this visit was 117 children. Proper ratios and supervision were observed.

During the visit, LPA obtained the following documents: ouch report for child 1, sign in sheets for 6/27/24, child roster, LIC 500, and updated LIC624. LPA interviewed Site Director who stated that child was doing good at this time. His cast will be removed on July 25, 2024. He was back from school and doing normal activity with other children. LPA interviewed both staff present when the incident occurred. Staff 1 and Staff 2 were on the play structure going around it.Sand was enough to cushion the playground and prevent injury to happen. Assistant Site Director called the parent, Emma Villavicencio to inform her of the incident. Parent took the child to Rady’s Children’s Hospital. The child was medically cleared with restrictions to return to the facility 48 hours later.

Supervision was in place, ratios were met, the facility responded appropriately and reported timely. This appears to be a random accident. No deficiencies are cited.

Exit interview conducted and report was reviewed with Director, Laura Clements. A notice of site visit was given and must remain posted for 30 days.
