25 | On November 17, 2025, at 12:30 p.m., Licensing Program Analysts (LPA’s) Cindy Meier and Raina Alexanian conducted an unannounced Case Management visit for the purpose of follow-up on an Unusual Incident Report that was submitted to the San Diego Regional Office. LPA met with Facility Representative, Site Director, Araceli Ongyan and discussed the purpose of the inspection and was led on a tour of the facility. There were eighteen (18) children present with five (5) staff members at the time of the inspection.
On 11/7/2025 around 12:35 p.m., Staff 1 (S1) was transitioning (10) children from the classroom to the hall bathroom. Due to child (C1’s) spitting behavior throughout the morning, S1 administered tape on C1’s mouth to prevent C1 from continuing to spit. Staff (S2) observed the tape on C1’s mouth, questioned S1 and advised the Director. According to the Director and S2, the tape was removed after approximately two (2) minutes. C1 testified to this action being conducted from S1. Director stated an Unusual Incident Report was emailed to Licensing on 11/13/25. Director stated staff received training in children’s Personal Rights and appropriate discipline on 11/31/2025.
During this investigation, LPA obtained facility records, S1’s written account of incident via email, incident report, video footage, and interviews from Director, S2 and C1.
Based on the video footage reviewed, S1’s admission in a written statement of the incident, and interviews, it is determined that inappropriate discipline was conducted from S1, and the Personal Rights of C1 were violated.
