25 | On 07/20/21 at 8:51 a.m., Licensing Program analyst (LPA), Rajani Goudreau made an unannounced Case Management inspection to follow up on an incident that was self-reported to the department. Upon arrival, LPA met with the assistant director Dilva Garcia and the director Maria Enriquez and disclosed the purpose of the inspection. LPA proceeded to tour the facility and observed the following ratios: Classroom #1 - six children and one staff, classroom #2 - four children and one staff, classroom #3 - five children and one staff, and classroom #4 - six children and one staff. Facility operates Monday to Friday from 6:30 a.m. to 4:30 p.m.
An unusual incident report (UIR) was submitted to the department on May 10,2021 alleging a possible personal rights violation that occurred on May 06, 2021. Interviews were conducted with staff members, parent, and children in care. Sign in and sign out sheets and the facility roster were reviewed. Facility video footage was reviewed of the classrooms and outside play area and no violations were observed. Based on the information obtained there were no violations.
No deficiencies issued during today's visit. Technical Violation advisory Note issued. LPA conducted an exit interview with the director. LPA discussed and provided the following to the director: LIC809, LIC9213-Notice of Site Visit, and Appeal Rights. The Notice of Site Visit shall be posted for 30 days from today's date. |
