25 | Licensing Program Analysts (LPA's) LaKesha Edwards and Joanne Domingo arrived at the facility on a Case Management-Incident visit to follow-up on an unusual incident report submitted by the facility via email on 8/18/2021. LPA's toured the facility, took census, interviewed staff and met with the Assistant Director Lizbeth Perez-Uribe.
The UIR is in regards to a child who tested positive for Covid on 8/17/2021. Parent called the facility site to notify that the child had tested positive and the child was current in care at the facility. The UIR states the child was immediately isolated, parent asked to pick the child up and 2 classroom cohorts were closed immediately and will reopen on 9/1/2021.
LPA's toured classroom 1A and 1B which reopened today 9/1/2021. All staff members in the classroom followed San Diego County Department of Public Health guidelines in wearing facial coverings properly covering their nose and mouths while inside the classroom. However, none of the children in care during the visit ages 2-3 years of age (12 in total) had any facial coverings on. This issue is addressed on a separate complaint investigation see (Complaint Control # 10-CC-20210824162802) dated 8/24/2021. Facility not following San Diego County Department of Public Health's guidance regarding children not wearing facial coverings will be addressed on the above mentioned complaint control number.
An exit interview was conducted and verbal technical assistance was provided to Assistant Director regarding following San Diego CDPH's guidelines and to encourage parents to follow in regards to providing facial coverings for children 2 years of age or older. A copy of this report was provided to Lizbeth Perez-Uribe Assistant Director.
