On 3/28/2022 at 9:30am, Licensing Program Analyst (LPA), Martha Malane conducted an unannounced inspection for the purpose of delivering findings for the above listed allegations. Upon arrival, LPA met with Director, Perla Zazueta and disclosed the purpose of the inspection. LPA proceeded to tour the facility and observed the following ratios: five (5) children with two (2) staff members.

On 1/10/2022 Community Care Licensing (CCL) received a complaint alleging allegations of: lack of supervision resulting in injury to infant in care, infant was left with a soiled diaper for extended period of time resulting in a diaper rash and parent was not notified of infant's injury.

During the course of the investigation, interviews were conducted with staff and daycare parents and records reviewed.

Due to conflicting information and a lack of supporting evidence obtained throughout the course of the investigation, the allegations are determined to be unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
