On 4/23/2025 @ 8:55AM Licensing Program Analysts (LPAs) Nancy Diaz and Jody Dye conducted an unannounced inspection to deliver the findings to the above allegations. Initial complaint inspection was conducted on 3/19/2025. LPAs met with Elizabeth Smith, Site DIrector. A tour of the facility was conducted. Observed present today were 29 preschool children with staff J. Rosas-Davis, P. Farmer, C. Richardson, K. Ziekman, N. Henry, S. O'Connell, S. Cadiero, S. Johnson and N. Howell.
Throughout the course of investigation interviews were conducted with the complainant, several employees and parents.
It was alleged that staff left day care children unattended when changing diapers. The diaper changing table was located inside the toilet, limiting the view of children in the classroom during diaper change. Staff interviewed indicated that additional staff are called in when they have to change a child's diaper. Staff denied that children were unsupervised during diaper change. LPA observed that the changing table has been moved out of the toilet and into the classroom.

It was alleged that staff inappropriately prepare food near changing tables. Staff who were interviewed denied allegation. Staff interviewed indicated that food are delivered in "buckets" then placed on top of the bookcase. Food is served immediately upon delivery.

Although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegations are found to be Unsubstantiated. Exit interview conducted and report wa reviewed with the Director, Elizabeth Smith. A notice of site visit was given and must remain posted for 30 days. There were no deficiencies cited today.
