25 | On December 1, 2022 at 9:30 a.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced case management inspection in reference to the licensee’s request for a capacity decrease. Upon arrival LPA met with Director Sara Gibbs and proceeded to tour the facility. There was one child with two staff members present. Appropriate ratio was observed. Staff members have the required background clearances and are associated to the facility.
The facility is currently licensed for a capacity of 20 infants, ages 12-24 months in room #4. The licensee requests to decrease the capacity to 9 infants, ages 12-24 months. On LPA Curtis’s inspection report dated 8/25/22, it was observed that classroom #4 had been altered and activity space had been removed. Classroom 4a was measured on 8/25/22 and is 325.50 sq. ft. which will accommodate 9 children. The space that was removed is now classroom 4b and is to be used for storage and/or a staff work area. A fire clearance was granted on 11/28/22 for 9 infants (ages 12-24 months) to use room 4a only. Classroom 4b is to be used as the work area/storage space.
LPA observed classroom 4a to be safe and free of hazards. There is age-appropriate furniture and equipment available for use. The outdoor infant play area was previously measured to be 2008.22 sq. ft. which will also accommodate the requested capacity.
No deficiencies are cited.
A license for nine infants will be sent for posting. An exit interview was conducted with the director and appeal rights (LIC 9058) were discussed. A copy of this report as well as a copy of the appeal rights were given to the director at the conclusion of the inspection. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit. |
