On the above listed date and time, Licensing Program Analyst (LPA) Kelly Gerth made an unannounced visit and met with Child Care Center (CCC) Director Liz Uribe to deliver the findings from a complaint made to Community Care Licensing (CCL) on July 23, 2025. The complaint CCL received stated the following allegations: Staff do not adequately supervise children while in care, Staff do not ensure that hazardous objects are inaccessible to children in care, Staff do not keep the facility clean and sanitary.
On July 25, 2025 LPA Gerth opened the complaint investigation at the CCC, conducted interviews with staff and collected evidence pertinent to the investigation. LPA Gerth conducted subsequential visits and/or interviews as well as collected additional evidence, on 07/25/25, 07/29/25 and 07/31/25. In total, LPA conducted interviews with 10 staff members (S1-10), the reporting party (RP) and an additional witness (A1). LPA also collected communication records, pictures, copies from staff files, observation notes, meeting agendas and a current LIC 500.

Regarding the allegation Staff do not adequately supervise children while in care, currently, the CCC operates as a Title 5 and Title 22 school and follows a lower staff to child ratio than CCL requires. During LPA’s visits on both 7/23/25 and 7/25/25 LPA observed that the facility was operating in ratio for Title 22 and Title 5 and did not observe cause for concern of inadequate supervision. The Licensing Program Analyst (LPA) observed that the additional staff present during both indoor and outdoor activities, in accordance with the CCC's adherence to Title 5 ratios, ensures that supervision cannot be deemed inadequate. Although there may be concern of some staff providing more adequate supervision than others, the CCC is currently meeting the minimum requirements for supervision under Title 22 regulations. Therefore, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED. Regarding the allegation Staff do not ensure that hazardous objects are inaccessible to children in care; during the course of the investigation, Licensing Program Analyst (LPA) inspected multiple classrooms throughout the facility. At the time of the inspections (07/23 & 07/25), no hazardous items were observed to be accessible to children in care. Interviews conducted during the investigation indicated that a staff member may have occasionally left hazardous items, such as adult scissors, within children’s reach. However, the LPA was unable to corroborate specific dates or times when these incidents allegedly occurred. Interview statements also confirmed that children did not access these potentially hazardous items and that such incidents were immediately reported to supervisory staff. Documentation provided by the Child Care Center (CCC) supports that staff were given refresher trainings on health and safety protocols. Additionally, supervisors reportedly conduct daily walkthroughs of classrooms to ensure ongoing compliance with safety procedures. Based on the information obtained, although the allegation may have occurred or is valid, there is insufficient evidence to determine whether a violation did or did not occur. Therefore, the allegation is deemed UNSUBSTANTIATED.
Regarding the allegation Staff do not keep the facility clean and sanitary. During LPA visits on 07/23/25 and 07/25/25 and interviews conducted during the course of the investigation, LPA found 3/12 persons interviewed stated they have personally observed a staff member to have subpar diapering and cleaning methods.

Allegations state a staff member has not been thorough in the cleaning of a child during diapering and sanitizing of the area including the changing table, post diapering. Interviews did reveal that these concerns were brought to supervisors, however LPA could not corroborate specific dates, times and events. LPA did find evidence of each classroom to have diapering and sanitation procedures posted, evidence of current diapering charts and staff meeting agendas where all staff have been given refreshers on cleaning processes. During the investigation, the CCC provided evidence of their Internal Health and Safety Director conducting unannounced visits and provided evidence of observation discussions.
LPA also found that supervisors walk through classrooms daily to conduct their own observations and help support classrooms in order to ensure that children’s health and hygiene needs are met. The center has also taken measures to help address any concerns that may arise between parents and or staff by making an anonymous concern and complaint QR code available for anyone to submit information to anonymously and efficiently. The CCC states they do not have any current or unresolved issues relating to health and safety concerns submitted through this complaint process. Therefore, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
A copy of this report, appeal rights and Notice of Site Visit were provided to Director Liz Uribe and was reminded that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
